This Informed Consent for Telehealth Services explains the nature, benefits, and risks of receiving healthcare through telehealth technology. Please read this document carefully before using Serena Health's services.

By using our services, you confirm that you have read, understood, and consent to receive healthcare services via telehealth as described in this document.

Nature of Telehealth Services

Telehealth is the delivery of healthcare services using electronic communications when the patient and healthcare provider are not in the same physical location.

Serena Health's telehealth services include:

  • Asynchronous Evaluation: Review of your medical intake form and health history by our clinicians
  • Electronic Prescribing: Transmission of prescriptions to our pharmacy partner when medically appropriate
  • Secure Messaging: Electronic communication with our healthcare team
  • Follow-Up Consultations: Phone or video consultations when medically necessary

Our services are specifically designed for evaluation and prescribing of longevity therapies (NAD+, peptides) and weight management medications (GLP-1 agonists).

Limitations of Telehealth

Telehealth has inherent limitations compared to in-person medical care:

  • No Physical Examination: Our providers cannot physically examine you, which may limit diagnostic capability
  • Technology Dependence: Service quality depends on technology; technical failures may delay care
  • Limited Scope: Not all conditions can be adequately evaluated or treated via telehealth
  • Information Limitations: Decisions are based solely on information you provide
  • Security Risks: Despite encryption, electronic communications carry some security risk

NOT FOR EMERGENCIES: Telehealth is not appropriate for medical emergencies. If you experience chest pain, difficulty breathing, severe allergic reaction, or any life-threatening condition, call 911 or go to the nearest emergency room immediately.

No Guarantee of Approval

Completing our intake form does not guarantee approval for treatment. Our clinicians exercise independent medical judgment and may:

  • Approve you for your requested treatment
  • Recommend an alternative treatment
  • Request additional information before making a decision
  • Decline to prescribe if treatment is not medically appropriate

If you are not approved, you will receive an explanation and will not be charged.

Your Responsibilities

As a patient receiving telehealth services, you agree to:

  • Provide Accurate Information: Give complete and truthful health history, including all medications, supplements, allergies, and medical conditions
  • Update Information: Notify us of any changes to your health status or medications
  • Follow Instructions: Use medications only as prescribed and follow all treatment instructions

  • Seek Emergency Care: Go to an emergency room or call 911 for urgent medical issues
  • Maintain Privacy: Keep your login credentials secure and protect your own health information

Provider Responsibilities

Our healthcare providers commit to:

  • Thoroughly reviewing your health information before making treatment decisions
  • Exercising independent medical judgment in your best interest
  • Prescribing medications only when medically appropriate
  • Providing clear instructions for medication use
  • Being available for follow-up questions and monitoring
  • Protecting your health information in accordance with applicable privacy regulations
  • Referring you to in-person care when telehealth is not appropriate

Follow-Up Care

Ongoing care is an important part of your treatment:

  • We may reach out periodically to check on your progress
  • You may request follow-up consultations at any time
  • Your provider may recommend dosage adjustments based on your response
  • You should maintain a relationship with a primary care clinician for general healthcare needs

Serena Health provides specialized services and is not a replacement for comprehensive primary care.

Right to Withdraw Consent

You may withdraw your consent to telehealth services at any time by:

  • Contacting our support team at [email protected]
  • Requesting discontinuation of treatment
  • Cancelling your subscription

Withdrawing consent does not affect:

  • Any care you have already received
  • Any legal obligations arising from previous services
  • Charges for orders already processed
